Written third party documentation of the disability is usually required before services can be provided, so a student should either bring appropriate documentation to his/her appointment with the Disabilities Coordinator or have it sent to the Disabilities Coordinator prior to the appointment.

Appropriate documentation may include: a statement from a medical professional; IEPs or other school records; evaluation reports from the Office of Vocational Rehabilitation (OVR), Blindness and Visual Services (BVS), or other service agencies that have documented the disability and recommended accommodations; or other appropriate records as requested by the coordinator.
